Your building representatives have this form if you need it. Please fill it out and give it to your building rep any time there is an incident that has caused you injury of any kind. Also, you MUST fill out the First Report of Injury any time there is an incident that caused you injury. This should be located in the main office. Do NOT let anyone tell you not to fill out the form if you are injured! It is very important to keep the administration aware of all injuries on school grounds, and injuries to staff.
